St. Luke’s LifeWorks is one of the largest organizations serving homeless people in southern Connecticut. We provide emergency, transitional and permanent housing, support services, youth services, education and employment training to more than 400 men, women and children in Lower Fairfield County each year. In addition to being homeless, the people we serve typically suffer from mental illness, substance abuse, and /or HIV/AIDS. Many further have severe cognitive impairments, a history of incarceration, little or no work history, and little education. All of our participants live at or below the poverty level.
Our mission is to break the cycle of homelessness by helping people achieve – and maintain – permanent housing and stability in their lives. Our principles are to be a results oriented, cost-effective, organization with a caring professional staff and dedicated volunteers who believe in individual human potential, personal accountability and mutual respect.
POSITION OPENING – Grant Writer (Part Time)
Position Description:
The Part Time Grant Writer is a new position. Working closely with the Chief Development Officer, the Grant Writer will manage all aspects of the grant applications process for family and corporate foundations and for City, State and Federal government grants.
Specific responsibilities include:
- Maintain the grants calendar and donor information within the Raiser’s Edge database
- Develop and write grant proposals, concept notes, and Letters of Inquiry
- Interview relevant stakeholders to develop effective proposals in support of both specific and general organizational priorities
- Work with relevant fundraising and technical staff to ensure that all grant requirements are met
- Work closely with finance on budgetary aspects of the grant proposal, including monitoring grant expenditures
- Ensure timely submission of grant applications and reports and progress reports to donors
- Draft acknowledgement letters for grants received
- Ensure that all proposals reflect the objectives of the grant maker; advance both organizations towards their mutual goals; include impact measurement; and adequately meet the funding needs of specific programs being supported by a grant
Qualifications include:
- Demonstrated track record in writing successful proposals
- Proven ability to prioritize and plan work activities
- Superior written and verbal communication skills
- Knowledge of Raiser’s Edge or similar system
- Ability to be flexible and handle multiple priorities under tight time constraints
- Bachelor’s degree required

POSITION OPENING - PROGRAM AIDE
Position
SLLW’s various housing programs all seek to take steps towards breaking the cycle of homelessness. Our staff collaborate to help move our clients towards obtaining an income (most often through employment), and then towards obtaining housing. Throughout the process, SLLW staff collaborates closely with our agency partners, including but not limited to: mental health, substance abuse, and medical treatment facilities.
Our goal is to successfully transition participants permanent housing in the community and to help them sustain this housing indefinitely. We accomplish this by addressing not just the physical housing part of homelessness, but all of the reasons a participant is homeless. This includes providing education and employment services internally and collaborating with a vast array of external partners to provide medical/dental care, mental health care, substance abuse treatment etc.
Duties
The Residential Program Aide will report directly to the Program Manager. Duties include, but are not limited to:
- Adherence to SLLW and contract policies and procedures.
- Working with program staff to ensure that the building and surrounding facilities are in compliance with health, safety and fire codes.
- Contacting Program Manager in the event of any emergency to ascertain best course of action.
- Consulting with other SLLW staff to ascertain best means of supporting participants.
- Liaising with SLLW external partners, including referring participants to services.
- Meeting individually with participants as needed to provide support in the areas of ADL’s, social skills, money management, employment, job retention etc. (in conjunction with SLLW programs).
- Other functions including monitoring client medication, preparing progress notes, service plans, incident reports, log books, etc.
Candidates should have: Bachelors Degree; three years of experience working with the homeless, mental health consumers, substance abuse, and/or families; strong computer skills (MS Windows, Outlook, Word, Excel); excellent clinical and organizational skills etc. Experience with Servicepoint/HMIS, DDaP, and RUUM a plus.

POSITION OPENING - CASE MANAGER – MCKINNEY HOUSE – HIV/AIDS PROGRAM
Position
SLLW’s McKinney House for HIV/AIDS is comprised of two components. The first is a 10 bed transitional housing program in which individuals have a 24 month length of stay. Extensions can be granted based upon the physical condition of the individual and the availability of permanent housing. The second is five units of permanent supportive housing for persons with HIV/AIDS. Persons in our transitional housing program often “graduate” to one of our permanent supportive housing units. Each person is assigned a single room and share common bathrooms, living rooms with TV’s. SLLW is one of the pioneers in HIV/AIDS in Connecticut. We began serving this population over 20-years ago.
SLLW is seeking a Case Manager to provide residential case management services in this program which is located at our Woodland campus in Stamford, CT.
The goal of the HIV/AIDS program is to successfully transition individuals with a history of chronic homelessness to permanent housing in the community and to help them sustain this housing indefinitely. We accomplish this by addressing not just the physical housing part of homelessness, but all of the reasons a participant is homeless. This includes providing education and employment training and collaborating with a vast array of external partners to provide medical/dental care, mental health care, substance abuse treatment as well as a range of other needs including, securing Social Security benefits, ADL’s, socialization skills, and employment.
Duties
The Residential Case Manager will report directly to the Program Manager. Duties include, but are not limited to:
- Responding to emergency and security situations.
- Collection and documentation of program participant fees.
- Adherence to SLLW and contract policies and procedures.
- Preparation of program reports.
- Serving as a back up to the Program Manager in his/her absence.
- Working with program and facilities staff to ensure the building and surrounding facilities are in compliance with health, safety and fire codes.
- Maintaining supplies inventory and distributing supplies to participants as needed.
- Leading weekly participant house meetings.
- Liaising with SLLW external partners, including referring residents to services and participating in collaborative treatment team meetings.
- Coordinating and running daily counseling groups that focus on ADL’s, social skills, motivation and related needs.
- Coordination of services with other SLLW programs, including the education, employment, housing coordination and retention programs.
- Maintaining a personal caseload of 10-15 participants, including all residential case management functions (weekly counseling sessions, preparing progress notes, service plans, incident reports, log books, etc.).
Candidates should have: MSW or related degree, experience working with the homeless, HIV/AIDS, mental health, and/or substance abuse populations preferred; strong computer skills (MS Windows, Outlook, Word, Excel); excellent organizational skills, motivated, team player, etc. Experience with DDaP & Servicepoint/HMIS a plus.
